| Description: 2024-25 After School Program Application - Children must be enrolled in Kindergarten to Grade 6 during the 2024-25 school year to apply. Note: This form is only for those that are NOT currently registered in the 2023-24 After School Program. Returning clients will be contacted directly regarding registration. Completing this application DOES NOT confirm you are registered for MAX Families After School Program. Your application will be reviewed and you will be placed on the waitlist or if there is space available you will contacted for follow-up. In March 2024 there will be a short period for returning clients and their siblings to register and then waitlist clients who filled out this form will be contacted in order to be offered any remaining spaces.
