| Description: 2024-25 After School Program Application - Children must be enrolled in Kindergarten to Grade 6 during the 2024-25 school year to apply. Note: This form is only for those that are NOT currently registered in the current After School Program. Returning clients will be contacted directly regarding registration. Completing this application DOES NOT confirm you are registered for MAX Families After School Program. Your application will be reviewed and you will be placed on the waitlist or if there is space available you will contacted for follow-up. Please see below list of our 2024-25 Afterschool non-licensed sites and schools serviced: (Important: Licensed sites [MacDonald Drive Jr. High, Southlands, Rotary Paradise Youth Community Centre & Paradise Intermediate] have their own application to register form.
